Within the carriers, there are three main plan types to choose from: Health Maintenance Organization (HMO), Point of Service (POS) and Preferred Provider Organization (PPO). The main differences between plan types are how much freedom you have when choosing providers and your amount of cost sharing. All plans include preventive care for free and comprehensive prescription drug benefits for children and adults.

Health Maintenance Organization (HMO)
HMO plans offer the flexibility to see any of the 35,000 participating providers in the BlueChoice network. The BlueChoice network is Maryland, District of Columbia and Northern Virginia. Outside of our network, only emergency medical services are covered.
Point of Service (POS)
POS plans offer access to care in the HMO network for the lowest costs and in the PPO network, which will be slightly more expensive.
Preferred Provider Organization (PPO)
Nationwide care (outside of Maryland, Northern Virginia and D.C.) is also available through the PPO plan type, but will cost you more. PPO plans offer the most flexibility. Care can be accessed from the BlueCross Blue Shield PPO National network of more than 40,000 providers nationally including the Mid-Atlantic region. Your patient responsibility will be higher if you see a doctor who does not participate with a Blue Cross and Blue Shield plan.

Kaiser Permanente Signature (HMO)
Through this type of plan, each member chooses a Kaiser primary care physician who arranges and coordinates primary, specialty, and hospital care. In most cases, members who require hospital services receive treatment at one of our primary hospitals, but Kaiser physicians may refer members to other hospitals as appropriate.
Kaiser Permanente Select
Through the select network, you will have coverage to a Kaiser Permanente primary care provider, or any provider who is contracted with Kaiser Permanente.
Kaiser Permanente Flexible Choice (PPO)
Kaiser Permanente Flexible Choice offers members three provider tiers and over 700,000 network providers nationwide. Tier 1 is their traditional HMO model, which will allow you to choose any physician practicing in our regional medical centers and affiliated community-based providers. This option carries the lowest out-of-pocket costs. Tier 2 is their national PPO network that includes 14,000 physicians and other health care professionals who are part of the CCN national PPO network. This option requires members to meet annual deductibles and pay copayments and/or coinsurance. Tier 3 is our Out-of-Network Provider tier, allowing you to choose any licensed provider not already identified in Tier 1 and 2. This Tier requires members to meet higher annual deductibles plus pay higher coinsurance.
